Adherence to prescription medicines has long been known to be a serious issue, hovering at only around 50% in developed countries according to the World Health Organization. As the former US Surgeon General Everett Koop famously observed “Medicines don’t work in patients who don’t take them”. Numerous tech-based solutions, including reminder apps, smart bottles, and automated dispensers have been developed over the last couple of decades but have barely made a dent in overall adherence. Only relatively labour-intensive approaches which involve significant patient engagement and support have proven to improve adherence meaningfully.

As the thousands of adherence studies done every year have shown repeatedly, poor adherence results from many very different factors including simple forgetfulness, hassle factor, physical or mental capacity, lack of appreciation for the importance of medications, concerns over side-effects, outright medical skepticism, or a challenging economic or social situation. Thus, to be effective any solution must respond to a patient’s individual circumstances, e.g. is a simple reminder adequate, or is a does a patient need education or ongoing support?

On top of the negative health outcomes caused by poor adherence, there are significant costs associated with not knowing what actual adherence was. Presently, doctors must rely on self-reporting or rough, ex-post data proxies. Patients who haven’t followed instructions are likely to be less than truthful about their behavior. This results in wasted medicines, and perhaps worse, leaves Doctors unable to determine whether undesired outcomes were the result of non-adherence or poor medical efficacy.
